Notice of Borderline Academic Performance

The purpose of this form is for the Assigner of Grade (Instructor of Record or Research Advisor assigning the grade) in a teaching, research, or journal club course to notify a student that their academic performance was on the edge of receiving an LP or NC grade.

It is a warning to the student that there are academic deficiencies in need of improvement, and that they should communicate with the assigner of the grade to ensure future performance is at the level expected of a Dartmouth graduate student.

(Instructor of Record, please complete. This form will go to the Guarini Registrar, who will keep this on file and send a copy to the program, assigner of grade, advisor as appropriate, and student)
